The Post-ICU follow-up guideline has been updated and thus also the module/web form on the Input Portal - Post-ICU follow-up to align with the national guideline from the Knowledge Management.

• The Input Portal - Follow-up after intensive care
  
  • The new forms and questions apply (and are visible) for patients discharged after 2025-01-01
• The guideline was published at the end of December 2024

Discharged from 2025-01-01, you can register 12-month contact but it is not mandatory. Manual will be published later.
